Home Based Medical Care Professional

Job Description
We are seeking a highly skilled Part Time Nurse Practitioner to join our Home-based Medical Care team in Dalton, GA. As a member of our team, you will play a critical role in bringing high-end medical service and compassionate care to complex, chronic patients.
Responsibilities:
- Establishes and maintains effective relationships with patients, caregivers, and healthcare providers.
- Provides comprehensive assessments and develops patient management care plans.
- Consults with Physician Leaders and coordinates with PCPs to ensure seamless care transitions.
- Performs episodic nursing care, including basic procedures and wound care.
- Participates in quality initiatives and HEDIS measure campaigns to improve patient outcomes.
Requirements:
- Certification through AANP or ANCC.
- Current, unrestricted NP licensure in State of Practice.
- Unrestricted RN licensure in State of Practice.
- Reliable transportation.
Compensation:$135,000 - $170,000 per year
