As a Nurse Practitioner, you will be part of our Home-based Medical Care team in Dalton, GA. Our organization, OptumCare, is committed to creating a seamless health journey for patients across the care continuum.

We are seeking an experienced Nurse Practitioner to join our team, providing high-quality patient care in the comfort of their own homes. You will work collaboratively with our integrated medical teams to address the physical, mental, and social needs of our patients.

Your primary responsibilities will include:

  • Establishing and maintaining effective relationships with patients, caregivers, and other healthcare professionals
  • Performing comprehensive assessments and developing personalized care plans
  • Providing episodic nursing care, including basic procedures such as IV placement, blood draws, and wound care
  • Participating in HEDIS measure campaigns and quality initiatives to ensure exceptional patient care

To be successful in this role, you should have:

  • Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Current, unrestricted NP licensure in the State of Practice
  • Actively hold, unrestricted RN licensure in the State of Practice
