Job Overview

This role offers a unique opportunity to gain supervised hours towards licensure in Mental Health Counseling.

PRIMARY ROLE: The Care Coordinator/Case Manager will work with a diverse group of clients facing substance use challenges alongside co-occurring mental health and medical conditions.

KEY RESPONSIBILITIES:
The Care Coordinator will actively engage with clients to assess their individual needs. This position involves enrolling qualified clients in a Health Home program, connecting them with community service providers, and supporting them in maintaining their appointments to meet their comprehensive needs, which may include physical and mental health, financial, legal, transportation, and employment services. The role requires collaboration with multidisciplinary teams to ensure clients' needs are met, aiming to enhance their overall health and reduce unnecessary emergency room visits and hospital stays. Additionally, the Care Coordinator is responsible for the accurate and timely submission of all relevant documentation.

WORK SCHEDULE: Monday to Friday, 8 AM to 4 PM

COMPENSATION: Annual salary of $49,000

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:

  • A minimum of two years of experience in providing mental health, social services, or substance abuse services to individuals with complex behavioral health issues is required.
  • A Bachelor's Degree in Social Work, Psychology, Sociology, Community Health, or a related field is mandatory. A Master's degree in Health and Human Services and CASAC certification is preferred.
  • Exceptional skills in engaging clients and families, along with proficiency in assessing and coordinating resources within family and community networks for effective patient care management.
  • Strong organizational, collaborative, and psychosocial assessment skills are essential.
  • A keen interest in participating in innovative initiatives aimed at enhancing care quality by addressing barriers to primary and specialty care, as well as preventing unnecessary Emergency Department visits and hospital admissions/readmissions.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) is required, with the ability to learn hospital scheduling systems, electronic medical records, and specific care coordination applications.
  • Bilingual proficiency in English and Spanish is preferred.
