Position Overview

The Case Manager plays a critical role in assessing and overseeing the care and supervision of residents at the Hope Harbor Center, a licensed residential facility for drug and alcohol treatment. This position involves the development and coordination of activities aligned with Evidence-Based Practices, entering patient data into relevant systems, identifying client needs, and enhancing their interests. Collaboration with the Clinical Program Manager and Counselors is essential to achieve optimal treatment outcomes.

Key Responsibilities

  • Facilitate and coordinate all scheduled in-service training sessions.
  • Manage a caseload of 12-14 clients effectively.
  • Ensure compliance with house rules and document significant incidents.
  • Conduct weekly case conferences with the program supervisor.
  • Develop and implement Evidence-Based Practice curricula in accordance with certification standards, working closely with the Clinical Program Manager.
  • Review client charts to ensure operational efficiency.
  • Verify that all clients have up-to-date TB tests and physical examinations.
  • Report maintenance issues to the Operations Manager and perform random room inspections for your caseload.
  • In emergencies, follow established procedures to ensure the safety of clients, staff, and the facility.
  • Possess knowledge of Drug Medi-Cal (DMC), understand the six dimensions of ASAM, and be capable of creating treatment plans and documenting case management.
  • Oversee all treatment services provided to clients at Hope Harbor Center.
  • Provide supportive counseling to clients and their families.
  • Develop community outreach initiatives and manage referral systems for mental health, medical, vocational, and additional therapeutic needs.
  • Network with community resources for referrals.
  • Collaborate with Veterans Per Diem Contracts DMC and IHP/DMH Grants.
  • Perform additional duties as assigned by the Director or Clinical Program Manager.

Working Conditions

Must be able to walk, stand, bend, squat, climb, kneel, and twist on an intermittent or continuous basis. Ability to grasp, push, pull objects such as files and drawers, and reach overhead. Proficiency in operating a computer, fax machine, and telephone. Must be able to lift up to 50 lbs.

Minimum Qualifications

  • Must be a certified or registered Alcohol and Drug Counselor (AOD) with current registration.
  • A Bachelor’s degree in social work or human services is preferred.
  • Experience working with parolees, mental health clients, DMC, and the homeless population is desirable.
  • Proficiency in Microsoft Word and Excel is required.
  • A valid California Driver's License with a clean driving record is necessary.

Skills, Knowledge & Abilities

  • Exceptional oral and written communication skills.
  • Strong organizational capabilities.
  • Ability to manage multiple priorities while maintaining attention to detail.
  • Familiarity with Salvation Army policies and procedures is preferred.
  • A criminal background check is required if working in proximity to children, in accordance with Protect the Mission policies.

Education

Bachelor's degree in Social Work is preferred.

Experience

Proficiency in Microsoft Word and Excel is required. Experience with parolees, mental health clients, DMC, and the homeless population is desired. Certification or registration as an Alcohol and Drug Counselor (AOD) is mandatory.

Licenses & Certifications

A valid Driver's License is required.
