Job Summary

We are seeking a highly skilled Pharmacist to join our team at Confluence Health. As a Clinical Pharmacist in our Ambulatory Care department, you will play a vital role in providing exceptional patient care and medication management services.

Key Responsibilities

  • Manage medication therapy and order appropriate labs based on published medical literature and Confluence Health guidelines.
  • Enter progress notes with additional information about medication therapy management decisions.
  • Communicate with patients, family members, and other healthcare providers to promote coordinated care.
  • Participate in department and organizational performance initiatives to improve patient outcomes.

Requirements

  • Bachelor of Science in Pharmacy (RPh) degree.
  • Licensed to practice in the state of Washington prior to start date.

Preferred Qualifications

  • Pharm. D degree.
  • Bilingual with ability to pass a bilingual fluency exam.
  • Three years of experience as a clinical pharmacist in the ambulatory setting or successful completion of a pharmacy practice residency with ambulatory care experience.
