Advanced Practice Provider

The Certified Advanced Practice Provider (APP) will provide comprehensive family medicine services to patients of Columbia Valley Community Health (CVCH). This role requires a strong commitment to patient-centered care, with a focus on delivering high-quality services in a fast-paced environment.
Key Responsibilities- Provide primary care services to patients, including routine check-ups, sick visits, and chronic disease management.
- Conduct thorough physical exams, take medical histories, and order diagnostic tests as needed.
- Develop and implement treatment plans, prescribe medications, and educate patients on self-care and disease management.
- Collaborate with other healthcare professionals, including physicians, nurses, and specialists, to ensure comprehensive care.
- Participate in quality improvement initiatives and contribute to the development of clinical guidelines and policies.
- Document patient interactions, medical histories, and treatment plans in a timely and accurate manner.
- Graduate from an accredited Nurse Practitioner or Physician Assistant program.
- Certification as a Physician Assistant (PA-C) or Nurse Practitioner (ARNP) in the State of Washington.
- Valid license to practice as an ARNP with evidence of prescriptive authority in the State of Washington or PA-C in Washington State or eligible to obtain licensure.
- Three years of experience working in a community health center or managed care environment preferred.
- Bilingual in English/Spanish preferred.
