Anticoagulation Therapist

Confluence Health is seeking a highly skilled Anticoagulation Therapist to join our team. As an Anticoagulation Therapist, you will play a critical role in the clinical management of patients on anticoagulation therapy.

1. Monitor medication therapy and perform ongoing assessments of patient risk factors to optimize therapeutic regimens.
2. Collaborate with the Physician Director or Pharmacy Manager to interpret medical records and test results, and adjust medication therapy accordingly.
3. Maintain competence in CPT coding and pass regular compliance audits.
4. Document patient care activities, patient and family teaching, and communication with the patient care team in the electronic medical record.
5. Demonstrate competency and proficiency in essential job functions by maintaining active credentialed status.
6. Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
7. Participate in department-driven practices, including provider huddles, journal clubs, and on-call scheduling.
8. Required to maintain active Basic Life Support (CPR) certification.

Required:

• Active Registered Nurse and ARNP licensure with prescriptive authority in the state of Washington.

Desired:

• Related experience, 3 years preferred.
• The following certification is preferred and will be required within 24 months of exam eligibility: CACP.
