Key Responsibilities:
  • Assess patients' daily physical, emotional, and mental health needs
  • Develop, implement, and evaluate individualized care plans in collaboration with residents' families and the healthcare team
  • Administer medications, manage diabetes, monitor patient responses, and adjust care plans as needed
  • Supervise CNAs, HCAs, and other care staff
  • Provide therapeutic interventions and manage crises for patients with behavioral health challenges
  • Assist patients with activities of daily living, ensuring their dignity and respect
  • Educate patients and families on managing dementia and behavioral health conditions
  • Maintain accurate and timely documentation of patient care in compliance with facility policies and regulatory standards

Qualifications:

  • Current and valid LPN license in the state of Washington
  • Minimum of one year of nursing experience preferred, with a focus on behavioral health, geriatrics, or dementia care
  • Strong clinical assessment and critical thinking skills
  • Excellent communication, interpersonal, and teamwork abilities
  • Compassionate, patient-centered approach to care
  • Ability to manage challenging situations with professionalism and empathy
