LPN Behavioral Health Specialist

We are seeking a skilled LPN to join our Behavioral Health and Dementia community, where your expertise and empathy will make a profound impact on the lives of our patients and their families.
As an LPN with our New Horizon Program, you will:
- Play a vital role in delivering specialized care to patients with behavioral health challenges and dementia.
- Assist in planning, implementing, and evaluating nursing care to uphold the highest standards of resident safety and wellbeing.
- Manage complex cases, offer emotional support, and work collaboratively with a care team to achieve successful outcomes.
Key Responsibilities:
- Work autonomously as well as collaboratively with the care team to deliver high-quality care.
- Assess patients' daily physical, emotional, and mental health needs.
- Develop, implement, and evaluate individualized care plans in collaboration with residents' families and the healthcare team.
- Administer medications, manage diabetes, monitor patient responses, and adjust care plans as needed.
- Supervise care staff and provide therapeutic interventions for patients with behavioral health challenges.
- Assist patients with activities of daily living, ensuring their dignity and respect.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals for a holistic approach to patient care.
Qualifications:
- Current and valid LPN license in the state of Washington.
- Minimum of one year of nursing experience preferred, with a focus on behavioral health, geriatrics, or dementia care.
- Strong clinical assessment and critical thinking skills.
- Excellent communication, interpersonal, and teamwork abilities.
- Compassionate, patient-centered approach to care.
