Job Summary

We are seeking a highly skilled Patient Advocate Representative to join our team at Conifer Revenue Cycle Solutions. As a key member of our revenue cycle team, you will play a critical role in ensuring that patients receive the financial support they need to access quality healthcare services.

Key Responsibilities
  • Conduct thorough interviews with patients and/or family members to determine eligibility for various government and non-government programs.
  • Record and maintain accurate documentation of all activities performed on patient accounts, including financial clearance and collections.
  • Perform financial clearance functions, including collections, and prepare accounts for Financial Assistance review.
  • Develop and maintain strong relationships with patients, hospital staff, and community partners to ensure seamless communication and coordination of care.
  • Conduct field visits to patient homes for skiptracing and/or assisting patients with documents.
  • Notify hospital case management, social services, and admissions staff of case screening determinations and outcomes via verbal and written communication.
Requirements
  • High school diploma or equivalent required; minimum 2 years of work experience in social services, hospital admitting, or a related field.
  • Working familiarity with Federal, State, and County programs, as well as proficiency in Microsoft Office applications.
  • Excellent oral and written communication skills, with the ability to work independently and prioritize multiple tasks with efficiency.
  • Bi-lingual preferred (Spanish).
Work Environment

This role requires the ability to sit and work at a computer terminal for extended periods, as well as the ability to walk through hospital environments and visit patients at bedside. Some travel may be required.
