  • Collaborate with Clinical Teams: Work closely with healthcare professionals to identify and address patient needs, utilizing various resources within the health system.
  • Community Resource Development: Proactively research and identify new community resources to support patients, enhancing their overall experience.
  • Professional Growth and Development: Participate in a growing professional network, contributing to the development of care guidance standards by disease state.
  • Patient Empowerment: Enable patients to communicate effectively with their healthcare team, taking an active role in decision-making processes.
  • Program Development and Evaluation: Contribute to the growth and expansion of our Care Guide navigation program, ensuring continuous improvement and evaluation.
Requirements
  • Bilingual Proficiency: Fluency in both Haitian Creole and English is required.
  • Technical Skills: Proficiency with personal computers and Microsoft Excel is preferred.
  • Excellent Communication and Interpersonal Skills: Strong organizational, communication, and interpersonal skills are essential for success in this role.
  • Education and Experience: A Bachelor's degree is preferred, but equivalent professional experience will be considered.
  • Immigration and Visa Sponsorship: Guideway Care does not provide immigration or visa sponsorship.
