The Youth Permanency Specialist will oversee a specific caseload of adolescents referred to Plummer by the Department of Children and Families (DCF). The primary responsibility of this role is to proactively guide the youth towards achieving permanency through the implementation of Plummer's Permanency Practice Model. Services will be provided using principles that are Youth-Guided and Family-Centered, including Family Search and Engagement, Permanency Readiness, and Youth Guided Teaming. This position may require occasional evening and weekend hours based on the unique needs of individual cases.


Key Responsibilities:
  • Conduct off-site meetings with each youth to introduce the permanency aspect of the Plummer Intensive Permanency Services program and encourage their active participation in their own permanency planning from the outset.
  • Collaborate with youth to identify parents, relatives, and other supportive adults they wish to connect with or re-establish relationships.
  • Utilize Family Search and Engagement strategies to locate parents and relatives, involving them in the permanency planning process.
  • Facilitate Safety Parameters Meetings to begin forming a supportive team.
  • Engage youth and families in Permanency Readiness activities, focusing on establishing and mending relationships.
  • Prepare youth for permanency and integration into family and community settings, helping them understand their experiences and the steps needed for reunification.
  • Equip families and supportive adults with the knowledge to provide lifelong support, offering psychoeducation on the needs of youth and a strengths-based perspective on trauma.
  • Implement Youth Guided, Family Driven Teaming to create a collaborative team with youth, families, and professionals, fostering shared decision-making to promote permanency.
  • Develop agendas for team meetings that drive the permanency process.
  • Educate stakeholders on the importance of permanency work and collaborate with systemic partners.
  • Actively participate in the youth's treatment team to advocate for and facilitate progress towards permanency.
  • Work with the DCF team to identify suitable permanency goals and address any barriers to achieving those goals.
  • Complete permanency-focused treatment plans within 30 days of intake and update every 90 days using electronic record-keeping systems.
  • Conduct quarterly progress reviews on electronic platforms.
  • Document all treatment interventions and interactions with youth and stakeholders in the electronic system.
  • Organize and lead Large Team Meetings at 30-day intervals and every 90 days thereafter.
  • Participate in external meetings related to youth on caseload as necessary.
  • Attend monthly Permanency Consults.
  • Perform additional duties as assigned by the Program Director.
Qualifications:
  • Bachelor's degree required; Master's degree with professional licensure preferred.
  • Minimum of an LSWA or LSW eligible to obtain LSW within 60 days of hire.
  • Strong understanding of DCF regulations, policies, and practices.
  • At least three years of experience working with children and families in the child welfare system.
  • Ability to engage youth and families authentically and creatively.
  • Excellent verbal, written, and organizational skills.
  • Physical ability to navigate stairways.
  • Visual and auditory acuity necessary for job functions.
  • Valid state driver's license, proof of auto insurance, and a reliable vehicle.
