About the Role:

The MDS Coordinator provides oversight of the Resident Assessment Instrument (RAI) process and conducts assessments and care plan coordination for our residents. This is a critical role that requires a strong understanding of the RAI process and the ability to work effectively with our interdisciplinary team.

Responsibilities:

  • Completes the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated time frames.
  • Coordinates scheduling the RAI process with the interdisciplinary team, ensuring timely completion of assessments and care plans.
  • Assesses residents through physical assessment, interview, and chart review, identifying their unique needs and care requirements.
  • Discusses resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff, to ensure comprehensive care planning.
  • Reviews information from hospitals, consults, and outside agencies, incorporating this information into the assessment and care planning process.
  • Coordinates, identifies, and/or initiates significant changes to MDS, ensuring accurate and up-to-date information.
  • Prepares for and conducts Prospective Payment System (PPS) meetings, maintaining MDS assessments per Medicare schedule and monitoring Medicare days and RUGs utilization.
  • Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements, ensuring compliance with industry standards.

Qualifications:

  • Licensed Practical Nurse (LPN) with AANC certification and RAC-CT preferred.
  • Strong knowledge of the RAI process, including the principles of the Prospective Payment Process (PPS), and experience as an MDS Nurse.
