Key responsibilities of the Home Health Registered Nurse role include:
- Providing home-based care to patients with a range of conditions
- Working with a team of healthcare professionals to develop and implement patient care plans

- Coordinating discharge from services and ensuring patient goals are met
To be successful in this role, you'll need:
- Active Registered Nurse (RN) License in Colorado
- Graduate of an accredited school or program of nursing
- At least 1200 hours of experience as an RN
- Healthcare CPR/BLS certification
- Home health care experience preferred
- Great organizational skills and ability to manage patients with chronic illnesses
