About the Role:
The Referral Care Coordinator serves as a vital bridge between the facility, marketing, and its clientele, as well as referral sources. With a dual focus on both nurturing client relationships and ensuring a smooth, continuous flow of referral communication and follow-up.
Responsibilities:
- Develop and maintain relationships with existing referral sources, such as hospitals, medical professionals, therapists, and community organizations for the purpose of streamlining patient care.
- Responsible for ensuring coordination of care between clients and referral sources are maintained as deemed appropriate by client.
- Act as the point of contact for referrers as it pertains to patient care.
- Coordinate and conduct regular check-ins with referral sources, for patient updates, addressing any concerns and gathering feedback.
- Maintain strong rapport with counselors and clientele.
- Follow up with clients post-treatment to gather feedback, testimonials, and success stories and to ensure successful transition into their next level of care.
- Coordinate with the facility's care and administrative teams to implement suggested improvements when applicable.
- Collaborate with the marketing team to share, develop, and implement strategies and improved systems monthly.
- Maintain accurate and updated records of all client and referral source interactions in the facility's EMR, CRM, and tracking systems.
- Attend discharge meetings to coordinate with referral sources.
- Assist with discharge planning and client discharge plan completion as needed.
- Act as a point of contact for coordinating Criminal Justice/Legal correspondence for the program.
- Responsible for setting up Provider/Referral Partners In-services for staff and patients.
Requirements:
- Excellent interpersonal and communication skills, with the ability to interact compassionately with individuals battling substance issues.
- Proficient in EMR, CRM software, and Microsoft Office Suite.
- Strong organizational and multitasking abilities.
- Strong ability to manage multiple relationships with various agencies.
- Ability to maintain confidentiality and act with discretion and empathy.
Education and Experience:
Bachelor's degree preferred. Experience in client relations, case management, marketing, or outreach, preferably within a healthcare or substance facility setting is preferred.
