Job Description

The Overnight Licensed Practical Nurse (LPN) will provide supervision and coordination of medication management, resident care delivery, and assistance with execution of activities programs in the Memory Care neighborhood. As an LPN, you will work closely with the Director of Nursing to set standards of nursing care and manage residents under the direction of a physician.

Key Responsibilities:

  • Assist in the initial assessment and development of Individualized Service Plans for incoming residents.
  • Interview and examine current residents to assess needs, monitor and record resident medical condition and behavioral changes, assist in the formal process to re-assess resident care needs, and recommend adjustments to care plans.
  • Supervise the ordering, delivery, storage, administering, and disposal of medications. Provide direct daily supervision of Medication Assistant staff activities.
  • Participate in discharge planning and the move-in and transfer of residents.
  • Evaluate and improve resident care policies and procedures, communicate them to staff, deliver in-services training, and reinforce proper performance on daily work.
  • Work cooperatively with residents, residents' families and representatives, community staff, physicians, consultants, and ancillary service providers. Act as liaison with the community and residents' supervising physician.
  • Maintain accurate and complete resident records, adhering to regulatory requirements, community quality standards, and resident rights. Ensure confidentiality of resident and community information.
  • Direct neighborhood team members and residents in executing emergency and disaster procedures during drills and actual events.
  • Provide daily report to the Memory Care Manager concerning community conditions, resident conditions, and needs for nursing supplies.
Requirements and Qualifications

To be successful in this role, you must possess:

  • A strong ability to communicate effectively with people of all ages while maintaining a positive attitude.
  • Excellent computer skills, including Microsoft Office and Property Management Systems.
  • A minimum of three years of experience in a similar position, preferably in senior living or home-care settings.
  • The ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, and governmental regulations.
  • The capacity to write reports, business correspondence, procedure manuals, and present information to various audiences.
  • A high school degree is required; a college degree is preferred.
Mental and Physical Requirements

This role requires:

  • Talking and hearing regularly.
  • Standing, walking, sitting, using hands to finger, handle or feel objects, tools, or controls, and reaching above, below, and at shoulder height frequently.
  • Lifting and/or moving up to 25 pounds, pushing a minimum of 200 pounds, and assisting in evacuating residents during emergencies.
