The successful candidate will facilitate Person Centered Planning, coordinate individualized services, and supervise Direct Support Professionals to ensure effective service provision.

Key Responsibilities
  • Evaluate and develop individualized plans with Direct Support Professionals to ensure compliance with New York State Home and Community-Based Waiver Services regulations and agency philosophies.
  • Supervise and train Direct Support Professionals to promote independence and community inclusion of individuals served.
  • Assist in the hiring and training process for Direct Support Professionals and ensure adequate supervision for the service needs of the individuals served.
  • Facilitate and support the development and maintenance of individual support networks through outreach to family members, community members, and professionals.
  • Monitor safety issues, coordinate medical supports, and respond to emergencies.
Requirements
  • Associate's Degree required; BA/BS preferred with at least 2 years of DSP experience.
  • Prior supervisory experience required.
  • Ability to establish and maintain working relationships with program participants, families, staff, and community agencies.
  • Ability to communicate orally and in written form.
  • Knowledge of Person Centered Planning.
