Job Summary

Under the direction of a Program Manager, you will supervise a team of Community Health Workers (CHWs) who work with high-risk patients to address complex medical and social issues. Your team will work collaboratively with interdisciplinary teams at multiple clinic sites across NYC H+H primary and specialty care clinics. Your key responsibilities will include:

  • Partnering with Program Manager and clinical leadership to refine and deliver the CHW Program
  • Participating in regular meetings with Program Manager and clinical leadership to review program progress
  • Assisting in the recruitment and hiring of excellent CHW candidates
  • Leading training and mentoring processes for CHWs, including developing individualized learning plans
  • Supervising CHWs at one or more clinical sites, providing support, coaching, and facilitating problem-solving
  • Conducting weekly 1:1 supervision meetings with each CHW to review patient caseload and provide feedback
  • Escalating issues to Program Manager or Clinical Leadership as needed
  • Facilitating weekly case conferences with the CHW team to review goal-directed interventions and develop patient recruitment strategies
  • Developing channels for communication between CHWs and clinic and hospital care teams
  • Managing patient outreach lists and tracking enrollment progress using the electronic health record (Epic)
  • Overseeing CHW patient intake processes and developing patient-centered, goal-directed treatment plans
  • Staying up-to-date on evolving patient resources and ensuring the CHW team is informed
  • Monitoring and tracking program progress and effectiveness of CHWs
  • Identifying and carrying out CHW program quality improvement initiatives
  • Participating in ongoing training activities related to the community health worker role and public health emergency response skills
  • Synthesizing observations and feedback from the CHW team to formulate program and policy recommendations
Requirements

1. One year of full-time experience in a government agency or community organization in providing community services to the public or assisting members of the community in obtaining community services; or
2. Completion of an approved six-month training program in community liaison work, plus six months of full-time experience as described above.

For Appointment to Level II, in addition to meeting the qualification requirements of Assignment Level I, appointment requires two additional years of full-time experience in counseling, community work, or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services, or maintaining liaison with schools, community organizations, or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology, or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above.

Preferred Qualifications

New York City residency; experience supervising staff in a healthcare setting, non-profit, or community-based organization; familiarity with email, word processing, and Internet usage; effective written and oral communication skills; proficiency in speaking a language other than English, preferably Spanish, Haitian Creole, Mandarin, Bengali, Russian, Cantonese, French, Arabic, Hindi, Urdu, Albanian, or Korean; ability and willingness to conduct home visits and outreach activities with assigned CHWs; four years of full-time experience in counseling, community work, or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services, or maintaining liaison with schools, community organizations, or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or education and/or experience equivalent to 1 above.
