Job Title: Travel Registered Nurse

At Sanzie Healthcare Services, we are committed to providing high-quality in-home health services to our clients and their families. We are seeking a skilled and compassionate Travel Registered Nurse to join our team.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide in-home healthcare services to our clients. The successful candidate will have a strong background in home health services and be able to work independently in a fast-paced environment.

Responsibilities:
  • Conduct in-home supervisory visits to monitor the care of clients with complex health needs
  • Review referrals from CCSP care coordinators and evaluate client needs during initial and subsequent visits
  • Develop and revise client care plans as necessary, communicating all revisions to the appropriate staff
  • Complete clinical records, including documenting progress notes and supervisory visit entries
  • Review, sign, and date all clinical record entries made by personal support aides
  • Communicate client care needs with other CCSP providers, members, member representatives, caregivers, and CCSP care coordinators
  • Make recommendations to CCSP care coordinators regarding client services
Requirements:
  • Minimum of two years experience in home health services or a related field
  • Working knowledge of the Rules and Regulations for Private Home Care Providers
  • Working knowledge of the policies and procedures of the CCSP, SOURCE, ICWP, and the GAPP Medicaid Waiver Program
  • License in the Practice of Nursing
