Job Summary:

The Home Health Registered Nurse will be responsible for assessing the health needs of our clients, developing and implementing individualized care plans, and providing ongoing care and support to ensure the best possible outcomes. This is a rewarding opportunity to make a meaningful difference in the lives of our clients and their families.

Responsibilities:
  • Assess the health needs of clients and develop individualized care plans
  • Provide ongoing care and support to clients in their homes
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Monitor and evaluate client progress, making adjustments to care plans as needed
  • Supervise and educate personal support aides and licensed practical nurses
  • Document client care and progress in a timely and accurate manner
Requirements:
  • Current and unencumbered license to practice as a Registered Nurse in Georgia
  • Minimum of two years experience in home health services or a related field
  • Ability to communicate effectively and work collaboratively with clients, families, and healthcare teams
  • Working knowledge of the Rules and Regulations for Private Home Care Providers and the policies and procedures of CCSP, SOURCE, ICWP, and the GAPP Medicaid Waiver Program
