Job Title: Travel Registered Nurse

At Sanzie Healthcare Services, we are committed to providing high-quality in-home health services to our clients and their families. We are seeking a skilled and compassionate Travel Registered Nurse to join our team.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide care to our clients in their homes. As a Travel Registered Nurse, you will be responsible for conducting supervisory visits, reviewing referrals, and developing care plans to meet the unique needs of each client.

Responsibilities:
  • Conduct face-to-face supervisory visits with clients to assess their health status and develop care plans
  • Review referrals from CCSP care coordinators and evaluate client needs during initial and subsequent visits
  • Develop and revise care plans as necessary, communicating all revisions to staff
  • Complete clinical records, including progress notes and supervisory visit entries
  • Review and co-sign documentation of LPN supervisory visits within 10 days of each visit
  • Communicate client care needs with other CCSP providers, members, and caregivers
  • Make recommendations to CCSP care coordinators regarding member services
  • Arrange for orientation and in-service training of staff
  • Educate clients, representatives, and caregivers about client health status, level of functioning, and healthcare needs
Requirements:
  • Minimum 2 years experience in home health services or a related field
  • Working knowledge of Rules and Regulations for Private Home Care Providers
  • Working knowledge of CCSP, SOURCE, ICWP, and GAPP Medicaid Waiver Program policies and procedures
  • License in the Practice of Nursing
