Job Title: Travel Registered Nurse

Job Summary:

We are seeking a skilled and compassionate Travel Registered Nurse to join our team at Sanzie Healthcare Services. As a Travel Registered Nurse, you will have the opportunity to work with our clients in their homes, providing high-quality care and support.

Responsibilities:

  • Conduct face-to-face supervisory visits with clients to assess their health status and develop care plans
  • Monitor and supervise the care of clients with complex health needs
  • Review referrals from care coordinators and evaluate client needs during initial and subsequent visits
  • Develop and revise client care plans as needed
  • Complete clinical records, including progress notes and supervisory visit entries
  • Communicate with other healthcare providers, clients, and caregivers to ensure seamless care

Requirements:

  • Minimum of two years experience in home health services or a related field
  • Working knowledge of Rules and Regulations for Private Home Care Providers
  • Working knowledge of policies and procedures of CCSP, SOURCE, ICWP, and GAPP Medicaid Waiver Program
  • License in the Practice of Nursing
