Job Summary

We are seeking a skilled Registered Nurse to join our team as a Complex Care Coordinator. In this role, you will be responsible for monitoring and supervising the care of our clients with complex health needs, ensuring that they receive the highest level of care and support.

Key Responsibilities
  • Conduct face-to-face supervisory visits with clients to assess their health status and develop personalized care plans
  • Review referrals from care coordinators and evaluate client needs to identify and assign the appropriate staff to provide care
  • Develop and revise client care plans as needed, ensuring that they are aligned with the client's goals and preferences
  • Complete clinical records, including progress notes and supervisory visit entries
  • Communicate with other healthcare providers, clients, and caregivers to ensure seamless care coordination
Requirements
  • Minimum of two years' experience in home health services or a related field
  • Working knowledge of the Rules and Regulations for Private Home Care Providers
  • Working knowledge of the policies and procedures of the CCSP, SOURCE, ICWP, and the GAPP Medicaid Waiver Program
  • License in the Practice of Nursing
