Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in providing direct nursing care to residents and supervising the day-to-day nursing activities in our long-term care and skilled nursing facilities. As a member of our team, you will be entrusted with ensuring the highest degree of quality care is provided to our residents.

Key Responsibilities
  • Coordinate resident care with other departments to ensure seamless delivery of services
  • Communicate effectively with employees, physicians, residents, and family members to address changes in resident condition
  • Perform administrative duties, including completing medical documents, reports, and evaluations
  • Admit, transfer, and discharge residents as required, ensuring a smooth transition of care
  • Evaluate and address resident abuse, ensuring a safe and respectful environment
  • Complete rounds to evaluate and assist in the development and implementation of various types of care needs on each resident
  • Provide treatments and care to medically complex residents in collaboration with the physician
  • Attend care plan meetings and provide updates as necessary to ensure appropriate resident care
  • Administer medications and treatments according to physician orders and standard of practice
  • Respond to all resident call lights, prioritizing resident needs
  • Coordinate with outside agencies, including hospitals, hospice, home health, lab, and x-ray services
  • Assist with training and supervision of nursing personnel, promoting a culture of excellence
  • Make rounds to evaluate quality of care performance and teamwork, ensuring quality service
  • Make recommendations to the nursing supervisor concerning employee dismissals, transfers, and other personnel matters
  • Respond to resident and family grievances, addressing concerns in a timely and respectful manner
  • Maintain confidentiality and rights of resident, family, employee, and facility information in compliance with PHI HIPAA policies and applicable regulations and company policy
  • Maintain a safe work environment, adhering to safe work practices
  • Demonstrate a passion for caring, evident in interactions with co-workers, residents, families, and visitors
  • Participate in QAPI and other quality improvement initiatives, driving excellence in care delivery
Requirements
  • Graduate of an accredited school of nursing
  • Wyoming LPN license
  • None required, but experience in long-term care and skilled nursing is preferred
Skills and Knowledge
  • Knowledge of computer systems, including Point Click Care, Google, and Microsoft Office
  • Working knowledge of long-term care and skilled nursing state and federal regulations (preferred)
  • Knowledge of nursing and medical practices, procedures, and standards of care
  • Leadership and supervisory skills, with a willingness to work harmoniously with and supervise other personnel
  • Patience, tact, and a cheerful disposition, with enthusiasm for handling difficult situations
  • Knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served
