Job Summary

The Substance Abuse Counselor is a vital member of our team at St. Joseph Center, working collaboratively with social service professionals and paraprofessionals to deliver high-quality care to our clients. This role requires a strong commitment to the mission of St. Joseph Center and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Develop and implement individualized case management service plans that address substance abuse issues, relapse prevention, and other risk factors that may impact client functioning.
  • Provide ongoing assessment and evaluation of client progress towards established goals.
  • Utilize a harm reduction model when working with clients, prioritizing their safety and well-being.
Essential Duties
  • Provide comprehensive case management services to clients, including linkage and brokerage to mental health, substance abuse, and community resources.
  • Respond to urgent requests for assistance from clients, providing crisis management and other related services as needed.
  • Participate in collaborative meetings with community partners and other stakeholders to ensure seamless service delivery.
  • Cultivate relationships with substance abuse treatment providers in the service area, promoting access to quality care for our clients.
Qualifications
  • Master's degree in social work, human services, or a related field, or a combination of education and experience.
  • Current and active CADAC or CAADE certification, with a strong understanding of harm reduction principles and practices.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
