Residential Nurse Educator

Mohawk Opportunities Inc. is seeking a dedicated and compassionate Residential Nurse Educator to join our team. As a key member of our healthcare team, you will provide medical advocacy, case management, and resource support to our clients living with mental illness, HIV/AIDS, and those who are homeless.
As a Residential Nurse Educator, you will serve as a resource for our clients in our four Community Residences, the Certified Apartment Program, and the Supported Housing Program. Your primary focus will be on ensuring that significant medical needs are appropriately addressed, and our clients receive the care and support they need to achieve stable community living and independence.
Key Responsibilities:
- Meet with new admissions to review medical history and ensure they are linked with a Primary Care Physician and have their annual physical.
- Establish and monitor medical plans of care for clients with significant physical health needs.
- Attend medical appointments with clients who require assistance communicating with their physicians or understanding their medical conditions.
- Act as a liaison with medical providers to obtain needed prescriptions, follow-up on questions or concerns, and schedule appointments.
- Coordinate post-discharge care for clients who have been hospitalized or had surgery.
- Consult with program staff to address medical concerns and provide education on case-specific medical issues.
- Facilitate the agency's Health & Safety Committee to review and make recommendations for clients with complex medical and/or psychiatric needs.
- Provide training to staff on medical topics and develop strategies to reduce excessive Emergency Room use.
- Complete nursing home referrals and oversee the system for staff supervision of client medications.
Requirements:
- RN with one year of experience in a residential setting, Bachelor's degree in nursing preferred.
- Computer experience with electronic records preferred.
- Valid and insurable driver's license, with a reliable vehicle for transportation.
- Ability to work independently and as a team member, with excellent communication and organizational skills.
Benefits:
- 40 hours per week, Monday-Friday, DAY TIME HOURS, NO ON-CALL.
- $2,884.62 biweekly, $75,000.12 annual equivalent.
