Residential Case Manager

As a Case Manager at Northern Rivers Family of Services, you will play a vital role in supporting clients in the Residential System. This position requires collaboration with clinicians to ensure the best possible outcomes for our clients. You will report to the Case Management Supervisor, Residential, and work onsite with community-based travel.
Key Responsibilities
- Arrange and attend home visits to assess client needs and develop personalized plans.
- Prepare transition plans to ensure a smooth transition for clients.
- Coordinate Independent Living Services to support clients in achieving independence.
- Act as the primary contact between Schools and Residential, facilitating communication and collaboration.
- Coach family visits to support clients and their families in achieving their goals.
- Complete various assessments, including FASP and SPR, to inform client care plans.
- Maintain accurate case notes and input them into the Connections database.
- Provide transportation for clients as needed.
- Maintain contact with DSS and family members to ensure seamless communication.
- Function as a contributing member of the interdisciplinary treatment team, attending dorm meetings and treatment review meetings.
Requirements and Education
- Requires a Bachelor's degree or commensurate level of education and related experience.
- Experience working with individuals in care systems is preferred.
- Must pass a physical examination.
