Clinical Care Manager RN

Amedisys, a leading provider of home health, hospice and high-acuity care, is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Clinical Care Manager. This role involves working with patients and families to develop and implement personalized care plans, promoting patient health and independence through teaching and rehabilitative measures.
CompensationThe estimated annual salary for this position ranges from $88,000 to $98,000, reflecting the cost of labor across various U.S. geographic markets. Compensation may vary depending on location, job-related knowledge, skills, and experience.
About the RoleAs a Clinical Care Manager, you will be responsible for performing patient assessments, collaborating with the care team to develop and implement care plans, and making referrals to other disciplines as indicated by the patient's identified needs. You will also supervise LPNs and HHAs, complete documentation timely and accurately, and regularly communicate patient progress to the clinical manager and care team.
QualificationsTo be eligible for this position, you must have at least one year of clinical experience as a Registered Nurse (RN). If you have less than one year of clinical experience as a RN, you must be approved by the VP Clinical. Current RN license, specific to the state(s) you are assigned to work, is also required. Valid driver's license, reliable transportation, and liability insurance are necessary for this role.
