About the Role:
This is an exciting opportunity for a skilled Registered Nurse to join our team as a Clinical Home Care Coordinator. In this role, you will be responsible for coordinating patient care, communicating with healthcare teams, and ensuring seamless delivery of services.
Responsibilities:
- Coordinate patient care activities, including scheduling appointments and arranging transportation.
- Communicate effectively with healthcare teams, including physicians, therapists, and social workers.
- Maintain accurate records of patient progress and communicate effectively with patients and families.
- Collaborate with other healthcare professionals to ensure seamless patient care.
Requirements:
- A current Registered Nurse license in the state of DC.
- A minimum of 2 years of experience in home health care or a related field.
- Excellent communication, organizational, and critical thinking skills.
- Ability to work independently and collaboratively as part of a team.
- Current CPR certification.
Estimated Salary Range: $70,000 - $90,000 per year.
