Job Title: Clinical Coordinator

At Open Systems Healthcare, we are seeking a highly skilled and dedicated Clinical Coordinator to join our team. As a key member of our clinical operations, you will play a vital role in supporting the delivery of high-quality patient care.

The ideal candidate will have a strong background in nursing, with experience in homecare settings. You will be responsible for coordinating client care, managing medical documents, and providing administrative support to our clinical team. Your excellent communication skills and ability to work in a fast-paced environment will enable you to thrive in this dynamic role.

Key Responsibilities:
  • Coordinate client care by assisting with case orientation, care insight review, and care coordination efforts.
  • Manage medical documents, including tracking order completion and ensuring signatures are captured.
  • Assist with post-fall and resumption of care visits, as well as discharge planning.
  • Maintain accurate clinical records and ensure compliance with Agency policy.

Requirements:
  • Graduate of an accredited school of professional nursing.
  • Homecare experience preferred.
  • Current and valid DC LPN license.
  • Evidence of Tuberculosis screening that meets CDC guidelines.
  • Current and valid CPR certification.

Compensation: $65,000 - $85,000 per annum, depending on experience. We offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
