Job Opportunity:
Affinity Care of Ohio invites applications for a Faith-Based Care Coordinator position. The selected candidate will play a vital role in delivering spiritual care services to patients and families/caregivers in a hospice setting.

Key Responsibilities:
* Develop and implement individualized plans of care that address the spiritual needs of patients and families/caregivers.
* Collaborate with the interdisciplinary team to ensure effective care delivery and achieve optimal patient outcomes.
* Provide direct spiritual care to patients and families/caregivers, including counseling, education, and support.
* Connect patients and families/caregivers with community resources and spiritual support networks.

Requirements:
* Bachelor's degree in religious studies or a related subject and master's degree in theology, divinity, or pastoral studies required; certification preferred.
* Minimum 1 year of experience in hospice care or a related field; experience in CPE highly valued.
* Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and faith traditions.
