Job Summary

We are seeking a compassionate and skilled Licensed Hospice Social Worker to join our team at Affinity Care of Ohio.

The successful candidate will possess a Master's degree in Social Work (MSW) from a school accredited by the Council on Social Work Education in the state of OH, as well as a minimum of one year experience in health care, with hospice experience being highly preferred.

Key Responsibilities:
  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment, communicating findings to the registered nurse and other members of the interdisciplinary group.
  • Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse, and plan intervention based on evaluation findings.
  • Assess for, and educate interdisciplinary group, on any special needs related to the culture of the patient and family.
  • Educate patients and families on, and assist in, preparation of advanced directives.
  • Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings.
  • Communicate with the physician regarding the patient's needs and report changes in the patient's condition.
  • Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
Salary and Benefits

This position offers an estimated salary range of $60,000 - $80,000 per year, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.

Requirements
  • Possesses and maintains CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
