About the Role

Southcoast Health System, Inc. is seeking a highly skilled Population Health Care Manager to join our team. As a key member of our healthcare operations team, you will be responsible for overseeing the development and implementation of population health management programs.

Key Responsibilities
  • Develop and maintain an evidence-based, highly effective care management team and practice model.
  • Coordinate provision of evidence-based, cost-effective case management services related to all aspects of population health management.
  • Work with the Executive Director to develop and maintain strong relationships with Network physicians.
  • Provide on-going direction and supervision of personnel assigned to the Southcoast care management populations.
  • Maintain a patient caseload as needed to adequately staff the department.
  • Oversee new OneCare and SCO delegation.
  • Build program, standard work and team in collaboration with the Executive Director.
Requirements
  • Bachelor's Degree in Nursing or equivalent is required.
  • Case management certification is preferred.
  • Experience in population health case management/care coordination is preferred; 5 years of related work experience including progressive and innovative leadership responsibilities are required.
  • Excellent interpersonal skills.
  • A licensed RN in both the Commonwealth of Massachusetts and the State of Rhode Island is required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
