About the Role

We are seeking a highly skilled and experienced Population Health Care Manager to join our team at Southcoast Health System. As a key member of our care management team, you will play a critical role in developing and implementing evidence-based care management programs to improve patient outcomes and reduce healthcare costs.

Responsibilities
  • Develop and maintain a comprehensive care management program that aligns with the organization's strategic goals and objectives.
  • Collaborate with healthcare providers, payers, and community organizations to ensure seamless care coordination and transitions.
  • Provide leadership and supervision to a team of care managers, ensuring they have the necessary resources and support to deliver high-quality care.
  • Develop and maintain strong relationships with network physicians and other healthcare stakeholders to ensure effective care coordination and communication.
  • Monitor and evaluate program effectiveness, making data-driven decisions to improve outcomes and reduce costs.
Requirements
  • Bachelor's degree in Nursing or equivalent required; case management certification preferred.
  • Minimum 5 years of experience in population health case management or a related field, with progressive leadership responsibilities.
  • Excellent interpersonal and communication skills, with the ability to work effectively with diverse stakeholders.
  • Licensed RN in both Massachusetts and Rhode Island required.
