Patient Care Director

We are seeking a highly skilled RN Patient Care Manager to oversee the day-to-day operations of our hospice site. As a key member of our team, you will be responsible for administering the clinical aspects of our hospice program and ensuring the provision of quality care to our patients.
Key Responsibilities:
- Manage the site in the absence of the Executive Director or Senior Patient Care Manager.
- Oversee clinical and administrative functions, including recordkeeping.
- Respond to phone referrals and inquiries in a timely and professional manner.
- Participate in the on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
Requirements:
- Registered Nurse with a minimum of an Associate's Degree in Nursing required.
- Bachelor's Degree in Nursing preferred.
- Three years' experience in direct patient care in a clinical setting required.
- Minimum one year of management/supervisory experience required.
- Two or more years' experience in hospice or home care required.
- Current license to practice nursing in the state of employment required.
