About the Role:

We are seeking a highly skilled and compassionate RN Patient Care Manager to join our team at Gentiva Hospice. As a key member of our hospice team, you will be responsible for the day-to-day operations of your assigned hospice site, ensuring the provision of quality care to patients and their families.

Key Responsibilities:
  • Manage the site in the absence of the Executive Director or Senior Patient Care Manager.
  • Oversee clinical and administrative functions, including recordkeeping.
  • Respond to phone referrals and inquiries in a timely and professional manner.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.

Requirements:
  • Registered Nurse with a minimum of Associate's Degree in Nursing required.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Current license to practice nursing in the state of employment required.
