Facility Overview

Telecare operates the King County Evaluation & Treatment (E&T) Facility, a licensed 16-bed establishment offering 24/7 intensive inpatient psychiatric care for adults facing mental health crises. We admit individuals aged 18 and older based on medical necessity criteria, either voluntarily or involuntarily.

Position Details

Full Time; DAYS Shift; Monday - Friday

The anticipated starting wage range is $28.38, with a full wage range extending to $33.80. Pay is adjusted based on geographic differentials, and starting compensation is aligned with relevant experience exceeding minimum qualifications.

Role Summary

The Care Transition Specialist is responsible for facilitating discharge planning services for clients and their families. This role includes coordinating and collaborating with various services and agencies, such as the Conservator's Office, eligibility workers, Public Defender, Public Guardian, Court Services, Third Party Utilization Review Departments, and multiple County Mental Health Services entities.

Key Responsibilities

  • Exemplifies the mission, purpose, values, and beliefs of Telecare in all interactions with stakeholders.
  • Oversees discharge planning responsibilities, including:
    • Coordinating assessments and discharges in collaboration with the treatment team.
    • Making suitable referrals for placement, case management, and aftercare services.
    • Identifying discharge planning needs and arranging follow-up care and financial assistance.
    • Updating the Treatment Team on social services and discharge planning.
    • Working with the Business Office to ensure client benefits are in order.
    • Developing and maintaining knowledge of community resources.
    • Completing necessary documentation for all discharged clients.
    • Coordinating services with client representatives.
  • Establishes and nurtures therapeutic relationships with clients.
  • Participates in community meetings and groups.
  • Adheres to Policies and Procedures in task execution.
  • Maintains thoroughness and availability to assist colleagues in urgent situations.
  • May assist with restraint of clients in cases of aggressive behavior and must complete crisis prevention training.

Qualifications

Required:

  • Two (2) years of professional experience in Social Work, Nursing, or a related field.
  • Must be at least 18 years of age.
  • Must obtain CPR, Crisis Prevention Institute (CPI), and First Aid certification upon employment or within 60 days and maintain current certification throughout employment.
  • All positions at Telecare are contingent upon successful completion of a post-offer physical examination, tuberculosis testing, acceptable criminal background checks, and verification of degrees or licenses.
  • Position requires a valid driver's license, motor vehicle clearance, and proof of auto insurance at the time of employment.

Preferred:

  • Bachelor's Degree in Social Work, Psychology, or a related human services field.
  • One (1) year of experience in mental health and/or public agencies.
  • Licensure as a Professional is preferred.

Skills

  • Understanding of psychiatric treatment modalities and community resources.
  • Ability to interface effectively with public and private agencies.
  • Strong written and verbal communication skills.

The employee may occasionally be required to reach, twist, bend, squat, kneel, and lift items weighing 25 pounds or less, as well as frequently sit, stand, walk, and perform simple grasping.
