Position Summary:

At Rosalie's Place, we provide transitional housing solutions aimed at assisting individuals who are either employed or on the path to employment, facilitating their journey from homelessness to stable, permanent housing. The Case Manager plays a pivotal role in guiding residents through this process by connecting them with essential services, advocating for their needs, and helping to eliminate obstacles that hinder their access to sustainable employment and housing.

The ideal candidate thrives in a collaborative environment, demonstrating quick thinking and adaptability in urgent situations. This individual possesses strong critical thinking skills to assist residents in identifying actionable steps toward achieving stability. The Case Manager must be capable of working autonomously while ensuring effective communication with team members. Key attributes include exceptional organizational skills, attention to detail, and the ability to foster relationships with community resources, all while providing encouragement and support to residents. This role requires a dynamic, resourceful, and dependable individual with excellent interpersonal skills and a commitment to serving vulnerable populations. The overarching goal is to equip program residents with the necessary tools and support to achieve lasting independence from homelessness.

Key Responsibilities:

  • Manage a caseload of up to 25 residents, conducting weekly meetings with each individual.
  • Conduct intake sessions with new residents, gathering essential background information and completing assessments.
  • Collaborate with residents to create and monitor individualized service plans (ISPs) that focus on skill development, job readiness, financial planning, and housing objectives.
  • Provide counseling and support to clients facing employment barriers.
  • Work with employers, training organizations, and community groups to facilitate job placement and training opportunities.
  • Assist residents with transportation to appointments as necessary.
  • Maintain accurate and timely records in case files and the Homeless Management Information System (HMIS), documenting interactions with residents, families, and social service agencies.
  • Engage in weekly case conferences to assess resident progress, collaborate with colleagues, and share resources.
  • Prepare comprehensive case notes and monthly reports for the program manager.
  • Coordinate with other program staff regarding resident interactions.
  • Assist residents in navigating community service systems, including attending appointments and providing hands-on support to overcome barriers.
  • Advocate with referral agencies to ensure residents receive appropriate services.
  • Facilitate problem-solving sessions with residents, conducting both group meetings and individual coaching as needed.
  • Perform additional duties as assigned by the program manager.
  • Exemplify and promote the Culture of Philanthropy within the organization, ensuring that volunteers feel valued and appreciated.

Qualifications:

  • Strong communication and organizational skills, with the ability to interact effectively with residents, staff, and volunteers.
  • Experience working with vulnerable populations in transitional housing or shelter settings is preferred.
  • Familiarity with community support services is advantageous.
  • Proficient in computer skills, particularly with Microsoft Office Suite, and capable of maintaining effective electronic communication and documentation.
  • A valid driver's license with a clean driving record is required.
  • Must obtain a Level One Fingerprint Clearance Card within 90 days of hire.

Education and Experience:

  • Bachelor's degree in social work or a related field is preferred, or equivalent experience.
  • Proficiency in standard computer systems (Microsoft Office).

Physical Requirements:

  • Ability to stand, walk, lift up to 50 pounds, sit, bend, and reach as required for the position.
  • Ability to read, comprehend, and communicate effectively in both oral and written forms.
