We are seeking a highly skilled and compassionate Case Management Coordinator to join our team at CVS Health. As a Case Management Coordinator, you will play a vital role in supporting our members in achieving their personal health care goals.

About the Role

This is a work-from-home position with travel required in Eastern and Central Maricopa county and Phoenix area to visit members. You will utilize your skills to coordinate, document, and communicate all aspects of the utilization/benefit management program.

Key Responsibilities
  • Apply critical thinking and knowledge in clinically appropriate treatment, evidence-based care, and medical necessity criteria for members.
  • Provide care coordination, support, and education for members through the use of care management tools and resources.
  • Evaluate members' needs and eligibility, and recommend an approach to case resolution and/or meeting needs.
  • Identify high-risk factors and service needs that may impact member outcomes and care planning components.
  • Coordinate and implement assigned care plan activities and monitor care plan progress.
Requirements

We are looking for a candidate with 2+ years of Case Management experience working with people who have been designated as having a serious mental illness (SMI) and working with people who are elderly or have a physical disability. You should have strong organizational skills, effective verbal and written communication skills, and the ability to work independently.
