The Case Manager will be responsible for coordinating healthcare decisions, ensuring treatment plans improve quality and outcomes, and promoting cost-effective care. The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work effectively with patients, families, and healthcare teams.

Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Coordinate care across the continuum, ensuring seamless transitions and optimal outcomes
  • Implement safe and effective discharge plans, ensuring patient safety and satisfaction
  • Collaborate with healthcare teams to promote cost-effective care and reduce readmissions
  • Provide education and support to patients and families, empowering them to take an active role in their care

Qualifications:

  • Current licensure in Social Work or a clinical discipline (RN preferred)
  • Previous experience in Case Management and Discharge Planning (preferred)
  • CCM Certification (preferred)
