Position Summary:

We are seeking a skilled and compassionate individual to join our team as a Facilitated Enroller at the Joseph P. Addabbo Family Health Center.

Under the direction of the Health Insurance Access program Director, the successful candidate will be responsible for completing all applications for the local departments of social services.

Key Responsibilities:

\u2022 Attend all trainings by the local departments of social services and NYS DOH to acquire, improve and maintain relevant knowledge and skills

\u2022 Conduct and participate in outreach events to effectively guide consumers through the application process

\u2022 Assist disabled, aged, blind, and non-MAGI populations by inputting all required household data on applications and accompanying documents

\u2022 Collect and copy required documentation

\u2022 Explain submissions and approval processes to consumers

\u2022 Contact local departments of social services as needed

Minimum Qualifications:

\u2022 Ability to handle confidential information according to JPA policies and procedures

\u2022 Ability to provide exceptional customer service

\u2022 Collaborative team player willing to provide assistance as needed
