Job Overview

POSITION SUMMARY:

The Housing Case Manager plays a crucial role in the provision and management of high-quality direct case management and stabilization services for individuals participating in the Veterans Housing Program at Housing Forward. This position is essential in facilitating housing advocacy and connection services aimed at ensuring clients maintain their housing and transition to lease responsibilities within a two-year timeframe.

KEY RESPONSIBILITIES:

  • Deliver continuous case management support to program participants by:
  • Collaborating effectively to assist clients in accessing resources, including employment and benefits, enabling them to take over lease responsibilities within two years.
  • Maintaining comprehensive documentation of all interactions with program participants, ensuring quality case notes are kept.
  • Creating and monitoring service plans in compliance with agency standards and funding requirements.
  • Establishing and nurturing service connections to support clients and their families, including building relationships with local service providers.
  • Linking participants to necessary medical, mental health, and other supportive services.
  • Providing transportation or arranging for clients to attend housing, legal, and medical appointments as required.
  • Offering ongoing life skills training that includes problem-solving and coping skills to enhance participants' independence and quality of life.
  • Developing a comprehensive understanding of community services and facilitating appropriate referrals for clients.
  • Conducting home visits to assist clients in achieving their goals and ensuring compliance with lease and program regulations.
  • Ensuring clients have access to and maintain eligibility for relevant entitlements.
  • Participating in required training sessions and meetings, both on-site and off-site.
  • Collaborating with the team to guarantee effective service delivery to all participants in the Supportive Housing Program.
  • Engaging in clinical case discussions and care coordination as necessary.

Program Compliance and Recordkeeping:

  • Regularly meeting with VA staff to receive referrals and track client progress.
  • Completing all designated reports and maintaining accurate documentation, including client case files and logs, in a timely manner as directed by the supervisor.
  • Preparing for and participating in monitoring visits conducted by VA staff.

Additional Responsibilities:

  • Collaborating with internal and external resources to expedite client housing placements, including outreach to individuals experiencing homelessness.
  • Encouraging client participation in the Tenant Club (Independent Living Skills Group) and facilitating group sessions as needed.
  • Working alongside the Housing Navigator to secure appropriate housing for clients and resolving any landlord-tenant disputes.
  • Ensuring clients capable of doing so pay the necessary program fees.
  • Leading at least one team project annually (e.g., ILS Group, House to Home, file Monitoring).
  • Facilitating Housing Team Meetings on a rotating basis.
  • Performing other duties as assigned.

QUALIFICATIONS:

  • Bachelor's degree in social services or a related field.
  • Master's degree is preferred.
  • Proficiency in word processing, data entry, and spreadsheet applications.
  • Bi-lingual candidates are preferred.
  • Proof of COVID-19 vaccination is required.
  • A smartphone is necessary for utilizing agency communication tools.
  • A valid driver's license and personal transportation are essential for this role.

CORE COMPETENCIES:

  • Exceptional case management abilities, with a capacity to work both independently and collaboratively.
  • Experience in interacting sensitively with individuals facing severe disabilities and homelessness, with a strong understanding of trauma-informed care and harm reduction principles.
  • Strong skills in interviewing, engagement, assessment, planning, and advocacy.
  • Demonstrated good judgment and the ability to work autonomously.
