We are seeking a skilled and compassionate Skilled Nursing Professional to join our team at Lakewood Continuing Care Center. About the Role:
In this role, you will be responsible for providing care to patients in a variety of settings, including private rooms and common areas. You will work collaboratively with nurses and other healthcare professionals to develop and implement patient care plans, ensuring that patients receive comprehensive and individualized care. You will also be responsible for maintaining accurate and detailed records of patient care, including progress notes and medication administration records.

Key Responsibilities:
- Provide direct and indirect patient care under the direction of nursing staff.
- Communicate and interact with patients and families, physicians, peers, and other healthcare members to maximize successful customer outcomes and unit function.
- Precept team members on job functions.
- Orient patients to hospital environment and routine.
- Document all aspects of patient care in a timely manner.
- Participate in unit quality improvement initiatives.
- Maintain a safe environment complying with Northern Light Health policies and procedures;
- Label and deliver specimens per policy;
- May perform other duties upon request.
