Job Title: Patient Care Coordinator

At Jackson Hospital, we are seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Responsibilities:
  • Perform routine nursing functions and diagnostic procedures to support medical plans of care.
  • Escort patients to exam rooms, collect vital signs, and record health maintenance data.
  • Draw blood for lab procedures, perform EKGs, and x-rays as ordered by physicians.
  • Administer injections, prepare patients for examinations, and assist with minor surgery and laceration repairs.
  • Record and review prescribed medications, assist with medication administration, and fulfill prescription refill requests electronically.
  • Maintain open communication with nurses, physicians, and supervisors, and perform all skills on the competency skill checklist.
  • Initiate, maintain, and perform patient education, and attain all clinical documentation for referrals and prior authorizations.
  • Complete all required documentation in the electronic medical record to meet Meaningful Use guidelines, and respond to patient needs and escalate messages to physicians when necessary.
Requirements:
  • High School Diploma or Equivalent, certification from an accredited Medical Assistant program preferred.
  • Previous work experience in a physician's office, with certification from an accredited Medical Assistant program preferred.
  • Excellent verbal and written communication skills, good telephone etiquette, and customer relations skills.
  • Ability to sit for prolonged periods, manual dexterity, and visual acuity necessary to utilize the PC and calculator.
