Home Health Care Coordinator

At (334900) Medi Home Health Agency, we seek a highly skilled Patient Care Coordinator to join our team. This rewarding role requires an LPN License and the ability to work in PA.
About the RoleWe are looking for a dedicated professional to serve as a liaison between our Agency and the medical community. The ideal candidate will ensure that patient care activities are timely, cost-effective, and of high quality.
- Establishing New Accounts: The successful candidate will establish new accounts and maintain existing ones within their assigned territory.
- Maintaining Account Profiles: They will complete and maintain weekly account profiles, account visit history, and account referral history in a profile book as instructed by DOM and/or Administrator.
- Account Visits and Liaison Services: The coordinator will maintain weekly contact and visits with accounts on their account list, developed with DOM and/or Administrator.
The Patient Care Coordinator will also act as a liaison between field staff and physicians in obtaining plans of care and appropriate modified orders.
Qualifications and SkillsTo be successful in this role, you must have:
- An LPN License and the ability to work in PA.
- A high school diploma or GED is required; a Bachelor's degree is preferred.
- Minimum of one year home care experience is preferred.
- Experience promoting community awareness related to home health and/or hospice is preferred.
- Reliable transportation and company-required auto liability insurance.
