About the Role

We are seeking an experienced Physical Therapist to provide high-quality services to patients in the home setting. The ideal candidate will have excellent communication skills and be able to work effectively as part of a multidisciplinary team.

Responsibilities

  • Deliver physical therapy services to patients in their homes, following physician orders and agency protocols.
  • Participate in case conferences and communicate with healthcare teams to ensure seamless patient care.
  • Maintain compliance with state and federal regulations, payer requirements, and agency standards.
  • Prioritize patient safety, infection control, and environmental safety measures.
  • Perform other job-related duties as assigned by supervisors.

Requirements

  • Active license to practice as a Physical Therapist in Missouri.
  • Home Health experience preferred but not required.
