Home Health Patient Care Coordinator LPN

We are seeking an experienced and skilled Licensed Practical Nurse (LPN) to join our team as a Patient Care Coordinator at Medi Home Health Agency, a division of Medical Services of America, Inc.
Based in Butler, PA, this role requires the ability to work in a fast-paced environment and collaborate with healthcare professionals to deliver high-quality patient care.
The estimated salary for this position is $65,000 - $75,000 per year, depending on experience.
Job OverviewThis is a full-time position that involves serving as a liaison between our agency and the medical community to coordinate patient care. You will ensure that patient care activities are appropriate, timely, and cost-effective.
You will establish new accounts and maintain existing ones within your assigned territory, complete and maintain weekly account profiles, and prioritize patient care issues and report them to the appropriate parties.
The ideal candidate will have an LPN license and be able to work in PA. A minimum of one year of home care experience is preferred, as well as experience promoting community awareness related to home health and/or hospice.
Responsibilities- Serve as a liaison between the agency and the medical community to coordinate patient care.
- Establish new accounts and maintain existing ones within your assigned territory.
- Complete and maintain weekly account profiles.
- Prioritize patient care issues and report them to the appropriate parties.
- Licenses: LPN License and ability to work in PA.
- Education: High school diploma or GED required, Bachelor's degree preferred.
- Experience: Minimum of one year home care experience preferred.
