Care Management Assistant Role

About the Position

The Care Management Assistant plays a vital role in supporting the licensed members of the Care Management team by providing administrative assistance with care coordination and transition planning efforts.

Key Responsibilities

  • Support efficient movement of patients to post-acute facilities, including Skilled Nursing Facilities (SNF), Long-term Acute Care Hospitals (LTACH), Inpatient Rehab Facilities (IRF), and Intermediate Care Facilities (ICF)
  • Receive task triggers to create referrals, transmit to post-acute care provider facilities via HIPAA secure platform, and trigger facility pre-certification initiation or initiate pre-certification as directed
  • Support department compliance to federal and state regulations by serving patient rights notifications related to Discharge Planning (Important Message from Medicare) and Utilization Management (Medicare Outpatient Observation Notice)
  • Work cooperatively with internal multidisciplinary teams, providers, allied health staff, as well as patients, caregivers, and visitors
  • Assist with arranging transportation to facilities or arranging follow-ups at the direction of licensed staff (RN TCC or SW)

Requirements

  • High school diploma or equivalent
  • Minimum of one (1) year experience in an administrative or business setting that requires supporting colleagues and working with data
  • Basic experience using a computer is required
  • Previous experience in healthcare preferred
  • Knowledge of medical terminology preferred
  • Minimum one (1) year experience working with electronic health record (EHR) preferred
  • Previous insurance and prior authorization experience preferred

  • Ability to work in a fast-paced environment with attention to detail
  • Ability to complete multiple tasks simultaneously
  • Maintains confidentiality of all patient information
  • Demonstrates basic computer skills as well as navigation and application of electronic devices and platforms
  • Knowledge of CPT and ICD-10 codes
  • Requires interpersonal skills sufficient to interact effectively with patients, their families, interdisciplinary caregivers, peers, subordinates, and supervisors
  • Excellent verbal and written communication skills
  • Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity

Physical Demands

  • Sedentary, exerts up to 10 lbs of force occasionally and/or a negligible amount of force frequently

Compensation

$15.91/hr - $19.09/hr

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location, are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers. Please keep in mind that the range that is listed is the full base salary range. Hiring at the maximum of the range would not be typical.
