Job Summary

CommuniCare Family of Companies is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for coordinating care services for our residents and patients.

Responsibilities:
  • Evaluate facility compliance with federal regulations and targets for utilization
  • Conduct weekly visits to facilities for intervention, audit, and training
  • Review resident clinical records for coverage, utilization, errors, assessments, and accurate transmission
  • Provide consultation and expertise in the area of specific federal and state reimbursement systems
  • Maintain current CPR certification
Requirements:
  • Current RN license in Ohio
  • At least 2 years of experience with Federal Medicare/Medicaid and Managed Care reimbursement system management in working with the MDS and RAI process
  • Senior MDS experience and/or multi-facility MDS experience preferred
  • AANAC certification highly preferred
  • Excellent written and verbal communication skills
  • Ability to multi-task and oversee functions at multiple facilities on a daily basis
