Job Overview

SUMMARY: The Patient Care Pharmacy Coordinator is responsible for engaging with patients through various communication channels to resolve both clinical and administrative concerns. This role facilitates collaboration among patients, pharmacy staff, healthcare providers, and external specialists to ensure comprehensive care. The coordinator also oversees the pharmacy's operational upkeep and assists in the efficient processing of prescriptions.

SUPERVISOR: Pharmacy Manager

KEY RESPONSIBILITIES:

Clinical Responsibilities:

  • Utilize electronic health records to schedule patient appointments and document interactions.
  • Evaluate medications for appropriateness, effectiveness, safety, and convenience while identifying any discrepancies.
  • Deliver health education and wellness information to patients.
  • Digitally archive patient documents into the electronic health record system.
  • Gather data to measure patient health outcomes.

Patient Engagement:

  • Provide ongoing support and care to patients.
  • Actively listen to and address patient concerns.
  • Coordinate care among pharmacy staff, healthcare providers, and specialists.
  • Notify relevant personnel of any patient-related issues.
  • Guide patients to community resources that address social determinants of health.
  • Assist in onboarding new patients.

Pharmacy Operations Support:

  • Enter patient and prescription information into the pharmacy management system.
  • Handle third-party billing and manage insurance claims.
  • Ensure accurate and timely prescription fulfillment.
  • Refer medication inquiries to the licensed pharmacist.
  • Adhere to current federal and state regulations.
  • Provide prompt service by assisting patients effectively.
  • Manage phone inquiries in a timely manner.
  • Process pharmacy transactions at the cash register.
  • Support the cleanliness and organization of the pharmacy.

Additional clinical and administrative responsibilities may be assigned by the pharmacy manager.

QUALIFICATIONS:

Required Experience and Education:

  • Must be at least 18 years of age.
  • Successful completion of a background check.
  • Basic computer proficiency is required.
  • Bi-lingual skills are preferred.
  • A minimum of one year of experience as a senior pharmacy technician is required.

CERTIFICATIONS:

  • Must be registered as a Certified Pharmacy Technician with the relevant state board.
  • Successful completion of either the ExCPT or PTCE is required.
  • Certification must be in good standing with the appropriate certifying body.

COMPLIANCE:

This position requires adherence to all organizational standards, including compliance programs and policies. Non-compliance may result in disciplinary action.

PHYSICAL REQUIREMENTS:

The physical demands of this role include regular sitting, using hands for tasks, and effective communication. The employee may need to lift up to 25 pounds frequently and occasionally up to 50 pounds, with reasonable accommodations available.

WORK ENVIRONMENT:

Employees may encounter various environmental conditions, including exposure to mechanical parts and outdoor weather. The noise level is generally low to moderate, with potential for high-stress situations.
